Nicholas Iverson, MD

Assistant Clinical Professor

Nick Iverson is UCSF Assistant Professor of Medicine, and works clinically as a hospitalist with ZSFG's Division of Hospital Medicine, as well as with the ZSFG Social Medicine Team as a consulting attending. He has particular interests in care transitions for vulnerable populations, as well as building community partnerships and engagement with health systems.

As Medical Director for Patient Flow in the ZSFG Department of Care Coordination (DoCC), Nick works closely with clinical teams and patients across ZSFG units and services along with the DoCC multidisciplinary team in order to meet patients' social and behavioral needs and ensure safe transitions of care. His expertise in food insecurity, social needs screening, and building community partnerships serve to help identify social needs and improve resources available to patients discharging from ZSFG.

Nick is the Medical Director for Food Bridge to Health (FB2H), a medically-tailored meal and grocery program with a mission to improve food security and justice on care transitions from ZSFG acute care units as a bridge to improved health and wellness. He leads the operational arm of an effort to expand universal social needs screening. He is also on the SF DPH SDOH Committee, working to operationalize social needs screening and interventions across the SF Health Network, and is a co-owner of the food insecurity A3.
